Create a Form Using Form Definition

To create a form starting from the Form Definition tab, do the following:

  1. Click the Create Form button in the top right corner of the screen. The Form Definition tab of the Create New Form page displays.
  2. Enter a unique form name in the Form Name field. Keep in mind that you will need to search for the Form Name of Final forms in Registration in order to associate the forms with a student.
  3. Select a purpose from the Form Purpose dropdown menu. Depending on the purpose you select, new required fields will display (see Form Purpose in the Item and Form Management Overview).
    • Note: For Final forms, you must complete the required fields before you can select items from the Items List.
  4. Once you have completed the required fields, click on the Items List tab.
  5. Click the Add Item button.
  6. In the Available Items window, select a content area from the Content Area dropdown.
  7. Search for items in the search field or add filters to narrow your search.
    • Note: For Final forms, Items will automatically be filtered to match the criteria you selected for Content Area, Grade, Standard, and Access Point on the Form Definition tab. You will not be able to filter outside of these criteria.
  8. Click ADD to add the item(s) to your form.
    • Note: For Final Forms, Items selected must equal exactly 5 points and be of the same standard.
  9. Click Add Items to Form to add them to your Items List.
    • Note: the items you select must equal a minimum of 5 points in the Points column for Final forms.
  10. If you want to reorder the items, click Reorder Items. In the Reorder Items List window, use the arrows to move items up or down, or change the number in the first box to reorder items. Then click Done to apply your changes or Cancel to discard your changes.
  11. When you are done editing the form, do one of the following:
    • Click Publish if you are creating a Final form and the form is ready for use. The form status will change to Published and you will not be able to edit the form.
    • Click Save if you want to save all of your information and keep the form in Draft status to continue editing.
    • Click Cancel to exit editing the form and discard any unsaved changes. The form status will remain as DRAFT and will contain any previously saved information.
